While I am a proponent of alternative treatments, particularly individualized therapy and healthy lifestyle changes, relying on alternative therapies can delay time to recovery. When symptoms impair function and cannot be controlled with alternative treatments, evidence-based medical management delivers scientifically proven results for most while alternative therapy can continue to provide support, adjunctive relief, and ideally long term psychological and behavioral growth.

There is great variability in human pharmacotherapeutic response. I often “start low and go slow” in medication for new patients, children, and the elderly. I see many medication sensitive patients and utilize a prolonged titration schedule in these patients whenever initiating or discontinuing medications. Less frequently, optimum dosing may exceed current recommendations.

Even with multiple adequate trials, not all patient symptoms respond to traditional pharmacologic and holistic approaches to management. For function impairing or life-threatening conditions, I may recommend more aggressive treatments such as deep brain stimulation, electroconvulsive therapy, or ketamine. Psychiatry treatment continually evolves, and I review current literature to stay apprised of the optimum treatment recommendations.
